    A study of inflation effects on an EOQ model for Weibull deteriorating/ameliorating items with ramp type of demand and shortages

    This paper deals with the effects of inflation and time discounting on an inventory model with general ramp type demand rate, time dependent (Weibull) deterioration rate and partial backlogging of unsatisfied demand. The model is studied under the replenishment policy, starting with shortages under two different types of backlogging rates, and their comparative study is also provided. We then use the computer software, MATLto find the optimal replenishment policies. Duration of positive inventory level is taken as the decision variable to minimize the total cost of the proposed system. Numerical examples are then taken to illustrate the solution procedure. Finally, sensitivity of the optimal solution to changes of the values of different system parameters is also studied

    Prospective analytical study of pattern of injuries in fatal cases of accidental fallautopsy conducted at Madurai Medical College

    Falls account for 11% of accidental injury deaths worldwide. 1/5th of global fall related deaths occurred in India. OBJECTIVE: To study the pattern of injuries in the victims of various level of accidental falls. STUDY DESIGN: Prospective Analytical descriptive study design. MATERIALS & METHODS: All victims of accidental falls who were autopsied at the mortuary of Government Rajaji Hospital, Madurai Medical College, Madurai from January 2016 & December 2016 were included in the study. RESULTS: • 210 people died due to accidental falls in the study period. The incidence of accidental fall for the year 2016 was 6.16% of which 54.8% were Ground level falls and 45.2% fall from height. There was a marked male preponderance in victims of both ground level falls & fall from height. • Most Vulnerable age group in ground level fall was 61-70 years and in fall from height 21-30 years age group. • Head injury was the commonest injury in both type of accidental falls and subdural hematoma was the commonly observed lesion. CONCLUSION: Elderly age group more vulnerable to death in ground level falls and working middle age group people vulnerable to fall from height in working places. Adequate preventive measures can prevent mortality due to accidental falls

    Domain formation of modulation instability in spin-orbit-Rabi coupled Gross-Pitaevskii equation with cubic-quintic interactions

    The effect of two- and three-body interactions on the modulation instability (MI) domain formation of a spin-orbit (SO) and Rabi-coupled Bose-Einstein condensate is studied within a quasi-one-dimensional model. To this aim, we perform numerical and analytical investigations of the associated dispersion relations derived from the corresponding coupled Gross-Pitaevskii equation. The interplay between the linear (SO and Rabi) couplings with the nonlinear cubic-quintic interactions are explored in the mixture, considering miscible and immiscible configurations, with a focus on the impact in the analysis of experimental realizations with general binary coupled systems, in which nonlinear interactions can be widely varied together with linear couplings.Comment: 11 pages, 10 figure

    Unusual localization of parathyroid gland inferior to the thyroid gland, absence of the isthmus of thyroid gland and intra thyroid thymic tissue - A case report

    Case report: Ectopic parathyroid glands are not rare and can be seen anywhere between mandibular angle and mediastinum. Absent of isthmus of thyroid was noted in a 36 year Sri Lankan male at postmortem. Lobes of thyroid were connected only by pretracheal fascia. The length of right lobe was 4.7cm and left was 4.8cm. Superior parathyroid glands (SPGs) were normal in position, left near cricothyroid junction and right just above first tracheal ring. The right inferior parathyroid gland (IPG) (8mm), tan yellow in color was observed just below lower pole of thyroid on anterior surface and extracapsular in position. Left IPG(1cm), brownish red color glandular tissue was situated at lower pole posterolateral and intracapsular in position. Histology revealed normal right IPG tissue and on left side thymic tissue with Hassall’s corpuscle present within thyroid capsule. Presence of ectopic thymic tissue within thyroid has been documented in children younger than 10 years. Agenesis of isthmus of thyroid may be due to a high division of thyroglossal duct. Thymus develops primarily from ventral and IPG from dorsal wing of third branchial pouch.Discussion: Occasionally thymus and parathyroids can be present in ectopic sites. Ectopic thymic tissue can be present in thyroid and be asymptomatic. Anatomical and clinical significance of absence of thyroid isthmus and ectopic sites of parathyroids is important for surgeons operating on the thyroid gland. Present case report is an attempt to highlight such.

    EXPERIMENTAL INVESTIGATION ON FLOW THROUGH A CONFINED RECTANGULAR CHANNEL MOUNTED WITH SQUARE BLOCKS

    Study on separated flow and step flow is subject of interest for many years for its wide industrial applications like cooling of electronic devices, heat exchangers, gas turbine components and other thermal equipments. Present experimental investigation is to report the influence of the block over rectangular channel in confined environment. Size of recirculation vortex, center of recirculation vortex for various Reynolds number with center to center distance between the blocks in the stream wise direction of the flow (Lx/Ly ratio) equal to 2, are the parameters reported when water is used as the working fluid. Outcome of this experimental study shows that, the size of the vortex increases with the increase of Reynolds number and the center of the vortex shifts in the direction of flow downstream of the blocks. Reynolds number considered for this study ranges from 800 to 1500 with a step of 100. Flow visualisation for the range of Reynolds number is presented

    Plasma Levels of Middle Molecules to Estimate Residual Kidney Function in Haemodialysis without Urine Collection

Middle molecules such as Cystatin C and β2-microglobulin accumulate in renal disease and plasma levels have been used to estimate kidney function early in this condition. We investigated their use to estimate RKF in patients on HD. DESIGN: Cystatin C, β2-microglobulin, urea and creatinine levels were studied in patients on incremental high-flux HD or hemodiafiltration(HDF). Over sequential HD sessions, blood was sampled pre- and post-session 1 and pre-session 2, for estimation of these parameters. Urine was collected during the whole interdialytic interval, for estimation of residual GFR (GFRResidual = mean of urea and creatinine clearance). The relationships of plasma Cystatin C and β2-microglobulin levels to GFRResidual and urea clearance were determined. RESULTS: Of the 341 patients studied, 64% had urine output>100 ml/day, 32.6% were on high-flux HD and 67.4% on HDF. Parameters most closely correlated with GFRResidual were 1/β2-micoglobulin (r2 0.67) and 1/Cystatin C (r2 0.50). Both these relationships were weaker at low GFRResidual. The best regression model for GFRResidual, explaining 67% of the variation, was: GFRResidual = 160.3 · (1/β2m) - 4.2. Where β2m is the pre-dialysis β2 microglobulin concentration (mg/L). This model was validated in a separate cohort of 50 patients using Bland-Altman analysis. Areas under the curve in Receiver Operating Characteristic analysis aimed at identifying subjects with urea clearance≥2 ml/min/1.73 m2 was 0.91 for β2-microglobulin and 0.86 for Cystatin C. A plasma β2-microglobulin cut-off of ≤19.2 mg/L allowed identification of patients with urea clearance ≥2 ml/min/1.73 m2 with 90% specificity and 65% sensitivity. CONCLUSION: Plasma pre-dialysis β2-microglobulin levels can provide estimates of RKF which may have clinical utility and appear superior to cystatin C. Use of cut-off levels to identify patients with RKF may provide a simple way to individualise dialysis dose based on RKF.Peer reviewe
